Release, Authorization and Waiver: I understand there is a risk of accidents happening when learning to sew. However, I also understand that the instructors will do their best to prevent accidents from happening during class instruction. In case of sickness or accident, I hereby give my permission to the medical personnel to order and/or perform any medical attention deemed necessary. I give consent for any necessary treatment when I (or my child) is in the care of a physician, hospital or dentist. Also, I accept financial responsibility if such treatment is necessary. I further understand that neither Esaie Couture Design School nor its workers can be held responsible in the event of accident. I assume all risk associated with my (or my child's) participation in sewing or related classes, and hold Esaie Couture Design School harmless for any injuries that may occur while attending this class (all locations), and all activites held by the instructors.
I also give my permission to Esaie Couture Design School to use any images taken of me, my child, or of his/her/my work, while attending any classes or events at Esaie Couture Design School.
No refunds will be issued after the first day of class. There will be no reduction of fees for absence, late arrival, late registration, withdrawal or dismissal.

Step #1: Fill out registration form, completely, for each student. Choose one or more classes. Read "Release & Waivers" and type in your signature. DO NOT Click "Submit" button yet.
Step #2: After filling out registration form, click "BUY THIS" to pay for each class choice in full, and to confirm your seat in class. A PayPal window will open up to complete payment. You may also choose to pay later with paypal's BILL ME LATER feature. (Don't forget to print your PayPal receipt and bring to class, as proof of full or deferred payment)
Step #3: After payment process is complete, return to registration form, click "Submit" button at bottom of page. Your place will now be reserved in class.
Each class is a one-time, 4 hour workshop
